Home Improvement Contractor License - Insurance Requirements


All contractors must submit the following insurance certificates:


  • Certificate of Liability Insurance
  • Workers Compensation Insurance Certificate C-105.2, U-26.3 or SI-12, GSI-105.2, or NYS Waiver Certificate CE-200
  • New York State Disability Insurance Certificate DB-120.1, DB-120.1, DB-155, or New York State Waiver Certificate CE-200


The City of Niagara Falls must be named as a Certificate holder on all insurance certificates. 


The following information shall be included in the certificate holder portion of the insurance form:

P.O. Box 69
Niagara Falls, NY 14302-0069


Effective December 1, 2008, CE-200 Exemption Form is available to download at the Workers’ Compensation Board website. Once you are on the website, click on Employers/Businesses >WC/DB Exemptions > Request for WC/DB Exemptions


Proceed to complete the form and at the end print out the CE-200 Exemption Certificate.


Please note that ACORD forms are not acceptable proof of New York State Workers, Compensation, or Disability benefits insurance coverage.
